Sick Notes

If you have been ill for 7 days or less you don’t need to see a Doctor.  You can complete a Self Certification form yourself.  This forms are available at the Surgery.  However some employers insist on a Doctors note regardless, if this is the case then there may be a charge.

If you have been ill for more than 7 days you will need a Doctor’s Certificate (Sick Note), for this on most occasions you will need to see the Doctor.

If you already have a Sick Note and you need this renewing please call the surgery after 1100 with the dates you require.  Please be aware this may not be done on the day you request it.  Give the the Doctor at least 48 hours to do this.
